Care Assessment carried out in hospital. 
Support and guidance with the discharge process including assessing reports from Drs, Physiotherapists, Occupational Therapists and Pharmacies. 
Support, advice and assistance with installation of new mobility equipment and Assistive Technology. 
Settling back in at home. 
Support with washing, dressing and personal care. 
Assistance and support to move safely around the home. 
Post-operative support including assistance with exercises. 
Management of Medication. 
Food and drink preparation. 
Domestic tasks such as cleaning, laundry and ironing. 
Transportation to appointments. 
Shopping. 
End of the day assistance and preparations for bed. 
And most importantly: companionship, a friendly listening ear and a smiling face.
